Community Outreach Worker
This is an opportunity for an enthusiastic and dynamic person to join our small, friendly team.
As a Community Outreach worker, you will be part of a team delivering services to adults with acquired brain injury. You will have experience of working with individuals with a disability or impairment.
The role is interesting and varied and will require you to provide support to individuals referred to our service, complete an assessment of need, keep concise and accurate notes, liaise with other professionals and report to the Services Director.
Our Community Outreach workers work independently in the community, supporting individuals and their families by offering information, advice and guidance, signposting, onward referral, and community integration. Knowledge of welfare benefits would be an advantage. Acquired brain injury affects individuals’ confidence and self-esteem and as a Community Outreach Worker you will need to be good at encouraging and motivating them to relearn skills and achieve greater independence.
